Do I add things like Ph.D (doctor) to the suffix field? Or where should it go?

Yes; that would be a suffix. Sometimes the stone with have Dr (as a prefix) and then also MD as a suffix. I just transcribe the way it is presented on the stone. Same goes for Rev, and various other religious designations, such as religious  "Fr. Francis Mulholland, SJ" Just use your best understanding and get the information in there somewhere.
